Agreement with and complete understanding of the benefits of the dental treatment that you need and desire form the basis for a financial arrangement between you and Dr. Sullivant's office.
Timely and affordable payment and treatment are the two goals and desired outcomes that determine the options offered and selected.
Dental treatment is an excellent investment in an individual's medical and psychological well-being. Many studies have found a close correlation between dental health and overall health. Financial consideration should not be an obstacle to obtaining this important health service.
We have found that our patients appreciate knowing exactly what dental financial responsibilities they will incur. Therefore, we inform our patients about our financial policies and responsibilities before we begin treatment. Knowing these policies ahead of time allows us all to arrange for the timely completion of your quality dental care.
We offer CareCredit, a healthcare credit card specifically designed to pay for treatments and procedures not covered by insurance. Your entire family can use the same card once you have been approved. CareCredit has two unique features: every CareCredit transaction is eligible for a No Interest If Paid in Full Within Promotional Period Payment Option or a Reduced Interest Payment Option, and you can use the card at all healthcare practices that "offer" CareCredit. You can apply by phone at 1-800-365-8295 or online at www.carecredit.com
A 5% prepay on over $1000 to the estimated patient portion of the treatment fee will be offered to those making payment in full by cash or check at least three days in advance of scheduled treatment for estimated charges over $1000.
PAY AT TIME OF SERVICE:
Payment is due in full for treatment provided at single appointments. For extended treatment procedures the total cost may be divided by the expected number of appointments and equal amounts paid per appointment. This must be arranged and agreed upon by the office manager in advance.
We accept MasterCard, VISA, Discover and CareCredit.
Insurance helps pay varying amounts of your dental expenses depending on your policy and the premium paid for it by your employer. It is one of the many savings options, but please understand, we have no control over the amount of savings it may provide you since it is based on a contract between you or your employer and an insurance company. Ultimately, all fees incurred in our office are the patient's responsibility. We will, however, submit all claims to your insurance company for you electronically, and will work to optimize your benefits.
Using the information you have provided us about your insurance plan, we will estimate what your portion of the treatment cost will be. We ask only for that portion on day of service. With a credit card guarantee, we will put the remaining balance (expected from your insurance company) on your account with a 60-day grace period. Balances not paid in that time period by you or the insurance company will be transferred to your credit card.
